Currently, Con-Pasi\u00f3n is looking to hire bilingual Community Psychiatric Support Treatment (CPST) Case Managers who provide support and coordination of care to individuals with severe and persistent mental illness, primarily in community settings. This person will assess client needs, develop and implement Individualized Service Plans (ISPs), and link clients to necessary community resources. This role also involves crisis prevention and intervention, which provides education and support to clients and their families, as well as monitoring their progress.<\/p>\n
