Request for Proposals/Applications 

As the official planning and service development agency for senior services covering 10 counties in Northwestern Ohio, the Area Office on Aging of Northwestern Ohio, Inc. (AOoA), is seeking proposals from qualified respondents for the provision of home and community-based services to be provided to older adults (individuals aged 60+) within the geographic area of:  Defiance, Erie, Fulton, Henry, Lucas, Ottawa, Paulding, Sandusky, Williams, and Wood Counties.  Services will be rendered under Title IIIB (Supportive Services and Senior Centers), Title IIIC (Nutrition Service) Title IIID (Evidence-Based Disease Prevention and Health Promotion Services) and Title IIIE (Family Caregiver Support Program) of the Older Americans Act (OAA) for January 1, 2027-December 31, 2030.  The AOoA is making available through this Request for Proposals (RFP) Older Americans Act, Nutrition Services Incentive Program (NSIP), Senior Community Services, State Alzheimer’s Respite, Title XX Social Services Block Grant and Lucas County Senior Services Levy funding.  Applications and bids for the services solicited by the AOoA will be accepted from any public, private non-profit or for-profit organization.  Services that may be funded include Personal Care, Homemaker, Housekeeping, Home Delivered Meals, Home Maintenance, Home Modification, Home Medical Equipment, Grocery Ordering and Delivery, Transportation, Socialization/Recreation, Health Screening/Medical Assessment, Home Injury Control/Assessment, Adult Day Services, Institutional Respite Care, Education/Training, Counseling, Benefits Counseling, Chore, Care Coordination, Congregate Meals, Consumer Finding/Outreach, Health Education (Evidence-Based Wellness Programs), Home Delivered Meal Assessment, Legal Assistance, Long Term Care Ombudsman, Nutrition Education, Emergency Response System and Supportive Services.
